England defender Gary Cahill helped Chelsea clinch the Premier League title on Sunday after the Blues beat Crystal Palace 1-0 at Stamford Bridge.
It marks the defender’s first Premier League winner’s medal since moving to Stamford Bridge in 2012.
Eden Hazard’s header on the stroke of half-time, after his penalty was saved by Julian Speroni, was enough to secure Chelsea's first league crown in five years.
Speaking during the post-match celebrations on the pitch, Cahill said: "You work day-in, day-out to get to this point. It's the best feeling ever.
"It's unbelievable."
In Sunday’s other game, England boss Roy Hodgson watched on as Joe Hart made a number of impressive saves, most notably to deny his international team-mates Harry Kane and Ryan Mason, as Manchester City strengthened their grip on an automatic Champions League place by beating Tottenham 1-0 at White Hart Lane.
The England No1 also played a part in City’s opening goal with a quick throw to launch a counter-attack which was finished clinically by Sergio Aguero. James Milner also completed 90 minutes for City and was booked.
England internationals Kane and Mason and Under-21 international Eric Dier all started for Spurs, but were unable to prevent the outgoing champions from claiming all three points in north London.
Meanwhile, U19s international Dele Alli helped MK Dons secure promotion to the Championship with a 5-1 defeat of Yeovil Town.
Alli was completing his loan spell with the Dons and will hook up with his new Spurs team-mates in the summer, having signed for Mauricio Pochettino's side in January.
