Two breaches of FA Rules – (i) breach of FA Rule E3 – it is alleged that on 20 April 2016 Lewis submitted a document to The FA, purporting to be a representation contract between him and Ashley Nadesan which had been forged and/or altered; (ii) breach of FA Rule F3 – it is alleged that Lewis failed to comply with a requirement under FA Rule F2 to attend to answer questions at a time and place determined by the Association.
